Once a patient at ECMC is referred to a specific therapy (physical, occupational, and/or speech), the patient is evaluated by a licensed therapist. The evaluation allows the therapist to establish a plan of treatment designed to help the patient move toward independence. The rehab professional works with the physicians, nursing staff, social workers and discharge planners to determine if the patient is ready for the transition from hospital to home.
An important part of this transition is family education and training. Each treatment plan is designed to provide education to the patient's family members and care givers. The education and training provide ways to assisting the patient with mobility, precautions following surgery, use of adaptive equipment and therapeutic exercises.
